Application Materials

The first step in becoming an HBEP Volunteer is to fill out an application. After your completed application is received and reviewed by our Volunteer Coordinator, you will be contacted to schedule an interview at the Bay. Applications can be downloaded and printed using the links below, or you can contact our office to have an application packet mailed to you.

Before submitting an application, please be sure you have reviewed the description of the volunteer opportunity you are interested in and that you understand the commitment, training, and other requirements associated with that position.

To download the application materials:

1. Use the links below to download a PDF* document of each of the forms
   
2. Print out each of the forms and fill them out completely
   
3. Mail all five pages to: Volunteer Coordinator

• your first and last name
  
• a complete mailing address (street, city, state, postal code)
  
• a phone number or email address where we can reach you
